Community Living disAbility Services Program

CTS provides consultative OT and PT services to individuals supported by Community Living disABILITY Services.

Assessments may be completed in the client’s home environment or Day Program. The clinicians will work with the client and their care providers to promote optimal function and independence.

 

Service goals include:

  • Enable individuals to safely remain in the community
  • Improve the quality of life and functioning of individuals and their caregivers/families
  • Recommend, provide equipment, alter methods of ADL/IADL
  • Decrease work related injuries to caregivers

 

CTS clinicians will complete assessments and follow-up as required to achieve meaningful functional outcomes for their clients, caregivers and families. Services may include:

 

  • Reviewing functional and safety concerns
  • Assessing short term or acute needs related to a new medical diagnosis
  • Assess physical function, movement, strength, balance, cardiac, respiratory status
  • Assess skin integrity, seating and positioning
  • Review transfers and mobility, including training of staff or care providers
  • Review equipment needs
  • Prepare letters of medical necessity for recommended equipment for third-party funders including: Public Guardian and Trustee, Disability and Health Supports Unity, Non Insured Health Benefits and others

 

Clinicians will prepare reports outlining their recommendations and communicate any assessment findings with the client’s health care team, caregivers and family.
