Home Care Program
CTS provides consultative occupational therapy (OT) and physiotherapy services (PT) to community-dwelling individuals and their families/caregivers through a Service Purchase Agreement with the WRHA Home Care program. An in-home evaluation by a CTS clinician may be accessed provided a client meets the eligibility criteria outlined below.
Eligibility Criteria
- Must have a 9 digit and 6 digit Manitoba Health card numbers
- Reside within the Winnipeg Health Region
- Require OT/PT assessment and short-term intervention
- Require an in-home or community-based assessment
- Cannot attend an appointment outside the home
- Not eligible for home-based therapy by another program
Service Goals
- Maximize clients’ quality of life and overall function of individuals, enabling them to remain in their home or community environment for as long as safely possible.
- Assess, recommend, and provide adaptive and specialized equipment to promote independence and safety
- Offer assessments and equipment to support care provided in the home by Home Care Attendants (HCA), Home Support Workers (HSW) and Home Care Nursing Staff.
- Facilitate safe and timely discharge from a healthcare facility
How to Refer
CLIENTS
Individuals receiving Home Care services may request a referral for OT or PT from their Home Care Case Coordinator.
Those seeking a referral for OT/PT services not presently receiving Home Care services may request a referral from their Primary Care Provider (ex: Family Doctor, Nurse Practitioner) OR they may self-refer by calling Home Care Central Intake to request an occupational therapy or physiotherapy referral.
