Personal Care Home Program
CTS provides consultative occupational therapy (OT) and physiotherapy services (PT) to some Personal Care Homes within Winnipeg and surrounding areas. OT/PT services are provided through a Service Purchase Agreement with the WRHA Long Term Care program.
The goals of rehabilitation services are to help residents achieve optimal function, safety and comfort while residing in a Personal Care Home. CTS rehab service goals may support a resident in any of the following areas:
Prevention
Reducing the occurrence of complications that may interfere with the resident’s optimum function.
Maintenance
Promoting maximum function at any given time in relation to resident’s capabilities.
Restoration
Promoting functional recovery that has declined as a result of an acute incident.
Safety
Developing a safe environment for the resident and care providers. CTS therapists will work with Personal Care Home staff to maximize the safety of transfers as well as mobility and exercise programs.
Comfort
Identifying strategies to increase the resident’s comfort and preserve the resident’s quality of life.
Adjustment and ongoing adaptation
Providing life enrichment strategies to correspond to the changing needs of each resident during his/her stay in the Personal Care Home.
Service Description
OT and PT services are provided through a collaborative process with the facility team members, family and the resident. While not exhaustive, CTS clinicians may support residents of Long Term Care through consultation in the following service areas:
- Physical and functional assessments
- Mobility assessments (walking, wheelchair, transfers)
- Falls prevention
- Walking programs
- General exercise programs
- Strengthening programs
- Orthotics
- Spasticity management (i.e. Botox consultation, management and follow-up)
- Pressure management (skin and wound support)
- Staff training for transfers and equipment
Speak to the staff of the Personal Care Home to request a referral for OT or PT.
